I think a previous crash on the node was with Captain America (regular)'s ult. Both were as Black Widow and/or Yelena would have been killed from the attack while in stealth. A third try using only basic attacks and not one-shotting either Widow or Yelena worked for me. None of the attacks on those 2 removed or flipped effects.
