In the past month, when I’m downshifting using the exhaust brake to slow down, it will jump to 4th gear and stay there. I’ll pull over, shut the truck off, restart and take off again and it’s back to normal.

OD is still used in 4th gear, but it only has to hold about 60% of the torque (versus 100% in 5th and 6th), so it sounds like it can still hold that OK. You should be able to drive it (if you use ERS "4" so it doesn't try to shift to 5th, and drive gently), but sooner or later it will probably fail completely.
